TMJ Arthrocentesis: Where It Fits in Joint Care

Arthrocentesis is one option within a staged approach to selected temporomandibular-joint disorders—not a universal treatment for jaw pain.

Temporomandibular disorders describe a group of problems affecting the jaw joints, chewing muscles and related structures. Similar symptoms can arise from different causes, so treatment should begin with diagnosis rather than a procedure.

Assessment comes first

A TMJ assessment considers the pattern of pain, joint sounds, mouth opening, locking, bite, muscle tenderness, previous injury and the effect on daily function. Imaging may be helpful when the history and examination suggest internal joint disease or another structural problem.

What arthrocentesis is

TMJ arthrocentesis is a minimally invasive procedure in which the upper joint space is accessed and irrigated. In selected patients, it may help reduce inflammatory mediators, release adhesions and improve movement. It is different from open-joint surgery and does not correct every cause of jaw pain.

Where it may fit

Many patients begin with reversible measures such as education, activity modification, appropriate analgesia, physiotherapy or a clinician-directed appliance when indicated. Arthrocentesis may be considered when symptoms and examination point to an intra-articular problem and conservative care has not provided adequate relief.

Benefits, limits and alternatives

Potential benefits, material risks, expected recovery and alternatives should be discussed for the individual patient. Outcomes vary, and published evidence is not equally strong for every TMJ diagnosis. The US National Institute of Dental and Craniofacial Research notes that arthrocentesis can help pain and mouth opening in some patients with disc displacement, while study results remain inconsistent.
